FirstNet - Subscriber Paid User Eligibility Verification Process

Congratulations on signing up for FirstNet!

Because you activated FirstNet service as a Subscriber Paid User¹ without prior verification by a qualified Primary User Public Safety Entity, you must successfully complete this eligibility verification process within 30 days after activation.²

Please provide the following information to help us verify your status as a valid Subscriber Paid User. Some individuals may receive an email asking for additional documentation.

If we can verify your eligibility, you will receive a confirmation email.

Accurate information is critical to the verification process. Locate your FirstNet phone number and account number to begin the submission. This information is located on the customer service summary document you received during the activation of your account.

Once you have located your FirstNet phone and account numbers and filled them in, please complete the rest of the form.

¹ Valid "Subscriber Paid Users" are individuals who are either (a) current employees of qualified Primary User Public Safety Entities, or (b) active, auxiliary personnel who are affiliated with, and provide services or perform functions on an occasional volunteer basis in the areas of law enforcement, fire protection, emergency medical services, or emergency planning and management (e.g., 911 call dispatching) for, qualified Primary User Public Safety Entities. Once verified, Subscriber Paid Users are subject to periodic re-verification.

² If you fail to complete this eligibility verification process or cannot be verified, AT&T will, in its sole discretion, suspend and/or terminate your FirstNet service (which will result in the full outstanding balance on your installment agreement, if any, becoming immediately due unless you return your device within 45 days of activation) or switch you to an AT&T consumer plan.
